DIABETES AND CELIAC DISEASE


What Is Celiac Disease?
Living with both diabetes and celiac disease can be a challenge, but it is not an impossible task.
Celiac disease is associated with the gluten protein in wheat, oats, barley and rye. The gluten damages the nutrient-absorbing lining of the small intestine (called villi). This damage causes diarrhea, weight loss and vitamin deficiencies—particularly deficiencies in B vitamins and especially folic acid. Poor digestive absorption leads to abdominal distention or bloating, muscle wasting and fatigue.
Once a gluten-free diet is started, the small intestinal villi, where nutrients are absorbed, begin to return to normal, a process that can take anywhere from one to six months.
In children, the trigger for celiac symptoms might be overeating wheat-containing foods at a party, sleepover or birthday splurge. Add a dose of emotional stress or a viral/bacterial infection, and gastrointestinal complaints can continue past the usual one to two days.
We do not yet fully understand why some people do not tolerate wheat and other gluten-containing foods. Further research is needed to solve this question of causation.

Nutrition Therapy for Celiac Disease
Steroid medication can suppress the intestinal symptoms that result from gluten consumption. However, most people with celiac disease will need to avoid all sources of wheat, rye, oats and barley until researchers gain further knowledge about how to customize nutrition therapy.
Corn and rice are the usual starch substitutes in the meal plan. Pasta products made from these sources are available. Millet and buckwheat do not contain gluten, although some doctors argue that they are often processed on the same grinding devices as wheat and therefore should be omitted from the diet because of cross-contamination.
A healthy diet for individual blood-glucose management can easily be made into a gluten-free diet with adequate freezer space and a list of reliable gluten-free food suppliers.
Many health food stores have numerous gluten-free items in the freezer case and snack food aisle. Twenty-five years ago, we had only rice cakes. Now rice cakes are a dieter’s snack food, and people with celiac disease can get frozen waffles, cookies and crackers.

Basics of Living Gluten-Free
That brings us to how much gluten is too much.
Diets containing as little as 2 to 5 grams of gluten per day (one slice of white bread contains 1 gram of gluten) have been reported to cause gastrointestinal changes. In one case, an 8-year-old boy whose only reported exposure to gluten was a Holy Communion wafer once a week had poor growth and partial villi atrophy.
Individual tolerances vary, but the rule of thumb is total avoidance if possible.
Learning the foods to avoid on a celiac diet is a lot like learning how to drive a car. Many foods can be hazardous to your health—just as road hazards can impair your driving. You need to stay alert, but you soon get used to it.
Here are some basic tips:
•           When ordering food in a restaurant, request that bread items be left off the plate.
•           The simplest way to avoid gluten is to follow a straightforward meal plan that also works well for diabetes management: for example, a serving of protein (broiled or baked), a starch such as a baked potato, steamed vegetables and fruit for dessert.
•           Snack on gluten-free foods later if you are still hungry. Birthday cake can be a big challenge—especially for the parents of a young person with celiac disease and diabetes. Today, gluten-free baking mixes make it possible to create a special cake for the occasion.

VEGETARIAN DIETS & DIABETES


Vegetarian diets are based on plant foods. Plant foods
include:fruits, cereals, vegetables, legumes (beans, lentils, peas) nuts and seeds. Dairy products (e.g. milk, cheese, yoghurt) and eggs are also included in some types of vegetarian diets.  

Most plant foods are low in fat and calories, and have no cholesterol. They are also high in vitamins and minerals.


WHY IS EATING VEGETARIAN GOOD FOR PEOPLE WITH DIABETES?       
Eating a vegetarian diet might be a sensible choice for people with diabetes. People who eat a vegetarian diet are less likely to have high cholesterol, be overweight or have high blood pressure. Vegetarians are also less likely to get heart attacks, blood vessel disease and osteoporosis. If a person has Type I diabetes they may need less insulin if they eat a vegetarian diet. If a person has Type II diabetes they may lose weight, which can help them manage their diabetes in a better way.  
       
HOW WILL YOU GET ENOUGH PROTEIN?        
If you decided to eat a vegetarian diet you may be wondering how you’ll get enough protein in your diet?  You are able to get all the protein you need by combining cereals (e.g. wheat, rice, barley, pasta, oats, rye, maize) with legumes, vegetables, nuts and seeds. You can also get protein from some animal foods, such as low-fat dairy products and eggs.  Semi-vegetarians also eat chicken, fish and shellfish on occasions.         

HOW DO YOU MAKE THE SWITCH?     
  
If you want to switch over to a vegetarian diet and can’t figure out how you can change the food in your present diet to a vegetarian diet, it may be advisable to speak to a dietician. They will be able to help you to plan a sensible vegetarian diet which will also fit in with your diabetic diet. It is, however, important to make sure you get all of the vitamins, proteins, minerals, fats and carbohydrates your body needs.          

If you’re thinking about eating a vegetarian diet and want to make some changes now there are some small things you can do. Get some vegetarian cookbooks so you can come up with new ways to make meals. If a restaurant you visit has a vegetarian menu try out some of the dishes. Start eating less meat, poultry and fish in your meals so you can get accustomed to not having them in your diet. While reducing your intake of meat, poultry and fish, it is important to increase your intake of legumes, cereals and vegetables to meet your energy and protein needs.

Diabetics, Why You Need the B's

Why You Need the B’s
 B-complex vitamins are essential for your body to convert sugar and starches to energy, a chemical process called carbohydrate metabolism. A shortage of any one of them can cause problems. Vitamin B6 deficiency has been linked to glucose intolerance, which is an abnormally high rise in blood sugar after eating.
Shortages of B vitamins can also lead to nerve damage in the hands and feet. Some studies indicate that people with diabetes develop less of the numbness and tingling asociated with diabetes-related nerve damage if they take supplemental B vitamins.
People with diabetes tend to use up their B vitamins. Also, poorly controlled diabetes causes these nutrients to be excreted in the urine. A supplement that includes 50 to 100 milligrams of most of the B vitamins is recommended.

Keep an Eye on Niacinamide

One B vitamin in particular, a form of niacin know as niacinamide, may actually help keep type 1 diabetes from developing in high-risk people. 
Type 1 diabetes is distinctly different from type 2, the more common type. People with type 1 develop antibodies that attack and destroy the cells in the pancreas that produce insulin.
Niacinamide seems to help protect insulin-producing cells from attack. Having enough on hand also helps your body produce more energy and facilitate chemical reactions.
Niacinamide is crucial in making a molecule that is central to the energy-production process. In large doses, niacinamide can also inhibit the formation of certain types of free radicals, the free-roaming, unstable molecules that set off destructive chain reactions in your cells.
Researchers from New Zealand did a two-year study using high doses of niacinamide. Among children with early signs of reduced insulin production, only 20 percent of those who took high doses of niacinamide progressed to type 1 diabetes. In contrast, 80 percent of a group of children with the same early signs of type 1 who did not take niacinamide did develop insulin dependence.
In a similar study with adults, only 15 percent of those taking niacinamide developed type 1 diabetes, compared with 40 percent of the adults in a group that didn’t take the vitamin.

Fabulous stuff, indeed. Vitamin B-6 has also been shown to help prevent heart disease, relieve morning sickness, provide relief from carpal tunnel syndrome and enhance brain function by assisting in neurotransmitter development.  If you’d like to include yourself in the group that has the highest intake of vitamin B-6, just add more bananas, spinach, avocado, poultry, fish, meat and beans to your diet. And it appears that an occasional cocktail or a glass of wine might help as well. Women in the Stockholm study who had plenty of B-6 in their diets and also drank two or more alcoholic beverages per week had an even greater reduced risk of colorectal cancer.

Vitamin E Improves Blood Flow & Nerve Velocity in People with Diabetes


 
Large doses of vitamin E may help normalize retinal blood flow problems and improve kidney function in people with diabetes, according to researchers at the Joslin Diabetes Centre in Boston.

 
      

According to the August issue of Diabetes Care, retinal blood flow increased to a near-normal rate and kidney function also improved in type 1's who took 1,800 IU of vitamin E.  
Researchers randomly assigned 36 patients with type 1 diabetes and nine nondiabetic control subjects to 1,800 IU of vitamin E per day or a placebo for four months. The vitamin E was natural d-tocopherol acetate dissolved
in edible vegetable oil. The placebo contained soybean oil. The subjects then switched pills for an additional four months. Practitioners were unaware which group had the real thing.   
Dr. George L. King of Joslin reports that after four months, retinal blood flow was significantly increased in type 1 patients after vitamin E treatment. He adds that vitamin E treatment decreased creatinine levels and
normalized kidney function in type 1 patients. He says that HbA1c was not affected by the vitamin E dosage in type 1 patients.  ”What we found is that a low dose of vitamin E is an antioxidant,” King says. “Higher doses have the ability to inhibit enzymes that are involved in diabetic complications.”   

Sixty Times the Recommended Daily Allowance  

In an editorial accompanying the report, Dr. Sushil K. Jain of the Louisiana State University Medical School in Shreveport says that the dosage of vitamin E used in the study was 60 times greater than the
recommended daily allowance of 30 IU per day, and that not much is known about potential problems that could arise from such a high level.   
King recommends that people with type 1 diabetes take 500 to 700 IU of vitamin E per day.

Vitamin E and Type 2 Diabetes     
 
In a previous study conducted on people with type 2 diabetes, researchers discovered that vitamin E supplementation could aid in the prevention of mild to moderate peripheral neuropathy in individuals with type 2 diabetes. According to the November 1998 issue of Diabetes Care, patients receiving 900 IU per day of vitamin E
for six months demonstrated increased motor nerve velocity when compared to a placebo.  



A Summary of Vitamin E Research for People with Diabetes      

In a January 1991 issue of Diabetes Care, it was demonstrated for the first time that vitamin E in patients with diabetes reduces protein glycoslation, which is a process where glucose attaches itself to the hemoglobin inside
your red blood cells, contributing to complications. Also, a study conducted by vitamin manufacturer Hoffman-LaRoche of Switzerland reported that doses as low as 200 IU of vitamin E significantly reducedglycoslation.
In other vitamin E studies:    

* The May 1993 issue of The American Journal of Clinical Nutrition reported that large doses of vitamin E improved blood glucose utilization and insulin action in people with type 2 diabetes. The study said that vitamin E is safe in amounts less than or equal to 800 IU per day, but could be unsafe at 1,000 IU or more per day. The researchers suggest that patients check with their health care professionals to see if they have any suggestions or opposition to taking 400 to 800 IU of vitamin E a day.       

* A November 1993 issue of Diabetes Care reported that vitamin E reduced plasma glucose, triglycerides, fatty acids and total cholesterol in patients with type 2 diabetes after a three-month test period. HbA1c levels were also significantly lowered. 

* A study in a 1995 British Medical Journal found that below-average vitamin E levels were associated with a 390 percent increase in the chances of developing type 2 diabetes.    

* In 1995, research conducted by Sven-Erik Bursell, PhD, of the Joslin Diabetes Center in Boston, suggested that 1,300 IU doses of vitamin E on a daily basis altered the development and progression of retinopathy.        

* Other studies have shown vitamin E’s benefit on cardiovascular activity. In a March 1996 issue of Lancet, researchers showed that vitamin E supplements reduced heart attacks by 75 percent in a group of 2,000
patients with heart disease. In a May 1996 issue of The New England Journal of Medicine, researchers showed that postmenopausal women who took vitamin
E were 62 percent less likely to die from heart disease. Also, a 1996 study of 2,002 patients with serious heart disease was presented at the annual meeting of the American College of Cardiology. After 17 months, 41 members of a placebo group suffered from nonfatal heart attacks, while only 14 people taking 400 to 800 IU of vitamin E daily suffered nonfatal heart attacks.
